چکیده

BACKGROUND Physical activity at moderate-high intensity is recommended to prevent lifestyle diseases. Patients with atrial fibrillation are at risk of a sedentary lifestyle due to fear of exercise-induced episodes of atrial fibrillation. The burden of arrhythmia can be reduced by physical exercise. The effect of exercise intensity on burden of atrial fibrillation needs to be studied further. METHODS AND RESULTS In a 12-week randomized controlled trial, 76 patients with paroxysmal/persistent atrial fibrillation were allocated to perform exercise at either low intensity or high intensity (50% and 80% of maximal perceived exertion, respectively). Primary outcome was burden of AF measured by daily electrocardiography-reporting during 12 weeks. Secondarily, change in maximal oxygen uptake (peak VO2) and 1-year hospitalization was compared between low and high intensity exercise. Sixty-three patients completed the follow-up. In the intention-to-treat analysis, we found no statistical difference in burden of atrial fibrillation between low and high intensity exercise (incidence rate ratio 0.742, 95% CI 0.29-1.91, P = 0.538). No serious adverse events were reported and there was no difference in hospitalization between the two exercise groups. Both exercise groups improved significantly in peak VO2 (low intensity: 3.62 mL O2/kg/min, SD 3.77; high intensity: 2.87 mL O2/kg/min, SD 4.98), with no statistical difference between-groups (mean difference: 0.76 mL O2/kg/min, 95% CI -3.22-1.7). CONCLUSIONS High intensity physical exercise was not superior to low intensity physical exercise in reducing burden of atrial fibrillation. HI exercise was well tolerated; no evidence of an increased risk was found for HI compared to LI exercise. Larger studies are required to further prove our findings. TRIAL REGISTRATION ClinicalTrials.gov NCT01817998.

Assessment of P-wave indices as atrial fibrillation predictors in psoriasis patients

Background: Psoriasis is a prevalent chronic T cell mediated inflammatory skin disorder. Recent studies have reported an increase in the incidence of arrhythmia in psoriasis patients who run an excessive risk of metabolic syndrome and cardiovascular diseases. P-wave dispersion (PWD) and duration are important electrocardiographic (ECG) markers employed to anticipate the risk of atrial arrhythmi...

The effects of Vitamin C on Inflammatory Markers and Atrial Fibrillation in Patients Undergoing Coronary Artery Bypass Surgery in Bushehr Heart Center: A Randomized Controlled Clinical Trial

Background: Coronary artery bypass graft surgery (CABG) with cardiopulmonary bypass pump may cause systemic inflammatory reactions by releasing cytokines which leads to multiple organ failure. Vitamin C has anti-inflammatory and antioxidant properties. The study aimed to determine the effect of vitamin C on inflammatory markers and atrial fibrillation in patients undergoing CABG. Materials and ...

تاثیر کوآنزیم Q10 در پیشگیری از فیبریلاسیون دهلیزی در بیماران تحت جراحی عروق کرونری

Background: Atrial fibrillation (AF) is the most common arrhythmia after cardiac surgery. AF leads to longer duration of hospitalization, thromboembolism, and impaired hemodynamics after heart surgery. One of the most important causes of postoperative AF, inflammation, and oxidative stress status. For this reason, it is useful to control the dysrhythmia. Coenzyme Q10 (CoQ10) as an antioxidant t...

بررسی تاثیر پریکاردیوتومی خلفی بر بروز فیبریلاسیون دهلیزی پس از عمل جراحی الکتیو بای‌پس کرونری

Background: Atrial fibrillation is the most common but benign arrhythmia following cardiac surgery. Although this arrhythmia is often self-limited and vanishes in about 24 hours upon surgery a number of arrhythmias might ensue among which some might be really life-threatening. A multitude of therapeutic modalities have been proposed for the prevention of this arrhythmia and one of them is poste...

Effect of High Intensity Interval Training on the Level of Atrial Fibrillation, Fibroblast Growth Factor 23 and Klotho Protein in Male Rats with Renal Failure

Introduction: Cardiovascular problems and atrial fibrillation is one of the most prevalent secondary consequences in hemodialysis patients. This study aimed to examine the effect of high intensity interval training on the level of atrial fibrillation, fibroblast growth factor 23 and Klotho in male rats with chronic kidney disease. Methods: In this study, 30 male rats Wistar (7-8 weeks) were ra...
